To be eligible to sit for the CNA exam in Texas, you must have two forms of proper identification with you. You will be required to present proper identification prior to both the written (or oral) examination and the skills evaluation. Photocopies of identification are not accepted. You will not be allowed to sit for the exam and your examination fee will not be refunded if you do not bring proper identification to the test site. You are required to bring two (2) forms of official identification:
- your signed, original Social Security card, and
- a valid photo-bearing identification.
At least one of these two forms of identification must be signed and the name on your identification has be the same as the one you used to register for the CNA exam. If your name is different, you must also bring proof of your name change to the test site which can be a copy of an official document such as a divorce decree or a marriage license.
