How can I reactivate my CNA certificate?

I was certified in Texas in 1999 and started working in a hospital 4 days after my certificate had expired. This escaped my notice and that of HR. I've been working in same hospital for 17 years but now am being required to be certified. I would like to know what I have to do in order to reactivate my certificate?

A certified nurse aide who has had 24 consecutive months during which he or she has neither performed nursing or nursing-related services nor has acted as a nurse aide for monetary compensation loses his or her certification and is removed from active status on the Texas Nurse Aide Registry. A person who has been removed from active status on the Nurse Aide Registry must successfully complete a new CEP or a new NATCEP to be restored to active status. Call the Texas Nurse Aide Registry at 512-438-2050 or complete form NAR-5528 and submit it to DADS for authorization to test. The form is available to download from here.
