How do I change my name on the Ohio CNA Registry?

I hold an Ohio license for STNA and I would like to know how to change my last name on the registry.

To change your name you have to contact the registry and provide them with copies of the following documents:

• driver’s license or state-issued ID,
• social security card,
• legal document such as a marriage certificate or divorce decree.

Call the registry at 800-582-5908 or 614-752-9500 or e-mail them at NAR@odh.ohio.gov to check that the address and fax number below are correct before sending the documents.

You may perform an online inquiry through the nurse aide registry website to find out if your request has been processed. Once your name is changed on the registry, you may contact D & S Diversified Technologies for a new card. You may call them at 877-851-2355 or go to their website to obtain a replacement registration card.
