Who should sign the CNA renewal papers?

I need to renew my CNA license and I need to know who signs the renewal application: an agency I work for currently or a facility i don’t work for anymore.

As far as I know, your renewal form may be signed by either your current or previous employer as long as they are a licensed health care facility or an agency licensed by the New Jersey Department of Health. For additional information on renewal and to check that this information is correct, you may contact PSI which is the New Jersey Nurse Aide and Personal Care Assistant Registry and Testing Service State Contract Vendor at 877-774-4243. You may also contact the New Jersey Department of Health at 609-633-9051 or by email at ltc@doh.state.nj.us (include the words "Nurse Aide" in the subject line) to obtain information on official regulations and guidelines regarding the nurse aide recertification process.

In order to be recertified as a CNA in New Jersey you must:

  • have a current, valid, nurse aide in long term care facilities or personal care assistant certificate,
  • have been employed performing nursing or nursing-related services for at least seven hours for pay, in a licensed health care facility or for an agency licensed by the New Jersey Department of Health, within the past 24 months from the date of expiration as specified on your nurse aide certificate,
  • not have had your CNA certificate revoked or suspended,
  • complete a criminal history check with fingerprinting prior to your certification expiration date.

The New Jersey Department of Health requires the renewal and updating of a nurse aide or personal care assistant listing on the Nurse Aide Registry, along with submitting to the fingerprint process, at least once every two years. A data renewal mailer will be mailed approximately 45 days prior to the expiration date listed on your nurse aide or personal care assistant certificate.
