How do I change my last name on my certification card?

I have recently got married and need to change my last name on my nursing assistant certification card. I live in Georgia and I am not up for renewal yet. How do I go about getting that done?

According to state and federal regulations, your name and address must be updated on the Georgia Nurse Aide Registry every time they change. For a change of name you need to complete and send the following form to the Georgia CNA registry: State of Georgia Nurse Aide Registry - Request a Change of Name. Include copies of all the required documents and follow all the instructions in the form.
