As far as I know, to obtain the complete Pennsylvania nurse aide registry list which includes all active, inactive and annotated nurse aides’ names and addresses, you should either send an email requesting the registry list to c-ncomplai@state.pa.us or send a written request to the Division of Nursing Care Facilities. The file will be sent as a .zip file via email or a CD via mail. The files can be imported to an Access document and sorted accordingly.
