I just got married and need to change my last name on the registry.

To change your name on the Illinois Health Care Worker Registry, you must provide them with a copy of your new driver's license or marriage license, a copy of your new social security card, give them your birth date and hand write out what you are requesting. You have to fax this to 217-524-0137 or mail to Illinois Health Care Worker Registry, 525 West Jefferson Street, Springfield, IL 62761.
