How do I renew my license and change my address?

I have a Georgia CNA license and am presently working in Philadelphia, Pennsylvania. My license will expire on June 30, 2017. I would like to renew my certification and change my address.

If your address, contact information, or employer has changed you need to fill out the “Change of Personal Information Request” form which can be found here. Do not use this form for change of name. A change of name may be requested by completing a “Change of Name” form that may be obtained here.
