How do I get my CNA license back in Louisiana?

I have worked as a CNA in Louisiana, but my job didn’t report it. I live in Shreveport LA, and my license expired 05-2016.

If you have the required work history but lost your certification because it has not been reported to the registry regardless of the reason, that work history must be submitted to the registry. Work history from providers or agencies who have a facility code, must be reported by mailing an official NAT-7 form to the registry or by electronic submission (eNAT-7) using LARS. The NAT-7 form is available by calling the Louisiana Nurse Aide Registry at 225-342-0138 or by sending a written request to P. O. Box 3767, Baton Rouge, LA 70821.

Work history from providers or agencies that do not have a facility code, must be submitted by the individual or the provider using letterhead and should be signed and dated by the administrator, their designee or an official of the Human Resources Office. The letter must include: your name, social security number, address, e-mail address and phone number, the date when you began working, the date you last worked, the nursing and nurse related duties you performed, and the address and phone number of the individual reporting the work history.

Work history from providers who employ nurse aides through staffing agencies must be reported by the provider who is licensed by the Department of Health and Hospitals, using the official NAT-7SA form or on letterhead.

Effective January 1, 2005, only work history from approved settings such as nursing homes, hospitals, home health agencies, personal care attendant agencies, assisted living programs, hospice agencies, respite agencies, supervised independent living agencies, adult day health care programs and intermediate care facilities for the developmentally disabled (ICF/DD) programs is accepted.

Duties performed in schools serving children with special needs, work in correctional facilities caring for patients, supervised independent living agencies, adult day health care programs, supported employment and intermediate care facilities for the developmentally disabled (ICF/DD) programs require a description of the duties performed to determine whether those duties meet the criteria of "nursing or nurse related".

The contents of the letter will be verified. Attach a copy of the W-2 forms which bear the name of the employer for the dates worked. If the W-2 forms are not available you should provide a front and back copy of a canceled paycheck for the period of time worked.

The option to retest is available to certified nurse aides whose certification is expired and is listed as "not certified" on the Louisiana Nurse Aide Registry. Aides are eligible to retest during the two year period immediately following the expiration of their certification.
